***IMPORTANT INFORMATION*** CONDITIONS OF ENROLLMENT FOR 2021

1. Score Day Camp’s full season begins June 30, 2021 and ends Friday August 21, 2021.

2. Score Day Camp reserves the right to terminate the stay of any camper if in the opinion of the Camp, it is in the best interest of the child or the camp. In such an event, it is understood that a proportionate (pro-rated) refund will be made.

3. Cancellations, in writing, for each registered camper prior to May 1 will receive a refund with the exception of a cancellation charge: – prior to March 31 the cancellation charge is $400 – April 1-28 the cancellation charge is $600 – on or after May 1 there can be no refunds made – days missed for any reason cannot be refunded; however, in the case of illness of more than two consecutive weeks: a written request accompanied by a doctor’s certificate is required and a 50% refund of the remainder of the registered session, after the two weeks, will be issued if the camper is unable to return. All refunds will be processed after the last business day of August 2021.

4. Score Day Camp cannot be held responsible for the loss, damage or theft of any of the camper’s belongings brought to camp. We strongly recommend that campers leave their personal belongings at home.

5. Score Day Camp cannot be held responsible for any claims for damages arising as a result of any accident, injury or otherwise sustained by the Parent or Guardian Signature above named child arising from participation in any camp activities.

6. Parents agree to provide their children with daily lunch, drink, snacks, sunscreen and all prescription medications (including epi-pens) where necessary, as well as all relevant supplies that pertain to such medications. Please note: all children carrying epi-pens will need to provide the camp with a second epi-pen to be kept in the camp office for emergency use only.

7. Parents agree to allow their child(ren) to participate in all camp activities, camp overnights (where applicable) and in any supervised activities not on camp property.

8. Parents agree to give Score Day Camp the authority to act on their behalf in case of emergency.

9. Score Day Camp reserves the right to cancel camps due to limited enrollment.

10. Parents agree that photograph and/or videos of their child(ren) may be taken and used for camp promotional activities.
